Change of Status Request for Current OCR Attorneys

If at any time you wish to make a change to your status or contact information with OCR, you must use this form to alert OCR to the requested change. Upon submission of this form, you will receive a confirmation and summary of your submission via e-mail. Depending on the nature of the request, OCR may contact you to discuss. OCR may not be able to accommodate all requests. If you wish to speak to someone about your request, please contact your staff attorney.

The best time of year to request to appear on a new OCR Appointment Eligibility List is during the applications cycle (and before the deadline for applications) when OCR is evaluating district needs. Please note that requests made March through June may be delayed while OCR is evaluating district needs and making contracting decisions for the upcoming fiscal year (July 1).
