OCR FY13 NEW ATTORNEY APPLICATION:

OCR FY13 RENEWAL APPLICATION:
Please click here to access Part I of the OCR FY13 Renewal Application. This part of the application requires the applicant to submit several documents in hard copy to OCR. Documents must be postmarked by Monday, May 14, 2012.
Please click here to access Part II of the OCR FY13 Renewal Application. This application must be completed online, and is intended for all attorneys who currently have contracts to provide OCR representation. This application must be completed by Monday, May 14, 2012.
Please Note: All attorneys who are awarded a contract with the OCR will be required to submit a waiver confidentiality for OCR to communicate with Attorney Regulation. A template will be provided when contacts are awarded /renewed.
